Understanding by Design
Below is a completely usable, and blank form on UbD. The document covers the entirety of the 2012-2013 academic year, makes use of bookmarks and hyperlinks, and has embedded English curriculum standards and objectives, as well as Gifted Benchmarks. Please bear in mind, there is a great deal of other features, and the teacher wishing to utilize the document will have to customize it to their needs. Before doing so, however, I strongly suggest a perusal of what the document has to offer.
In the original "blank" version below, please bear in mind that it is not truly blank. An English teacher wishing to alter it should add units and suggested study appropriate to the grade level they are teaching. For non-English teachers, the entirety of the objectives/documents should be revised to suit the needs of the class.
With this said, let me throw in a couple of disclaimers. First off, the format used is an accepted compacted form of UbD. It is not, of course, the only form of lesson plans available or acceptable. Teachers wishing to learn this format (whether they are to use it or not) can go to http://pixel.fhda.edu/id/six_facets.html for a fairly comprehesive look at the methodology.

My second disclaimer is one of conventions and presentation. Due to my desire to get this document out as efficiently as possible, so that as many teachers as possible can make use of it, anyone who spends any amount of time at all will note that page numbers need revision, formatting is inconsistant, and the enumeration within the text (most notably in the daily lesson plans) leaves much to be desired. Instead of correct these minute details, I present the document as imperfect as it is to be used at the teacher's discretion. When time permits, I will do my best to alleviate such irritations. I apologize in the forefront for any errors. My hope is that this format will make everyone's overtaxing job a bit less burdensome.
I ask only that you make it your own. Add lessons; add protocols, add materials links, how to videos, do as you wish....
